index email not defined/validation error

Hello All:

Receiving the following error when trying to run the PHP script on my site:

Notice: Undefined index: email in c:\Inetpub\wwwroot\grandiose.php on line 247

Notice: Undefined index: emailmethod in c:\Inetpub\wwwroot\grandiose.php on line 248

Warning: Cannot modify header information - headers already sent by (output started at c:\Inetpub\wwwroot\grandiose.php:247) in c:\Inetpub\wwwroot\grandiose.php on line 251

A validation error occured. If this problem persists, you will need to generate a new script.

Any help would be appreciated. Looking forward to participating in the project!!!

This is PHP 5.1.1 running on Windows Server 2003 Enterprise and IIS6.
